Job Summary
Provide hands-on personal care, hygiene, and Activities of Daily Living (ADLs) including safe transfers, mobility support, and assistance with bathing, dressing, and oral hygiene. Support nutrition through meal preparation and feeding, manage homemaking tasks like light housekeeping and errands, and facilitate community engagement, appointments, and transportation. Monitor basic health and safety, implement care plans, and proactively document observations while collaborating with the care team. Requires active DODD certification, CPR/FA, and a minimum of one year in-home care experience. Immediate start available for weekday daytime shifts with a 30-day build-up to full-time hours.
Required Qualifications
- Minimum of 1-year paid experience in home care
- Active DODD Certification
- Willingness to travel to/from client throughout Seneca County (30-45 minute radius)
- Willingness to travel to clients up to at least a 45-60 minute radius
- Immediate availability
- Weekday - daytime schedule
- Exceptional on-time and attendance record
- Consistent availability aligned to chosen schedule type
- Active CPR + FA certification

- Willingness and ability to perform required physical demands
- Consent + Ability to pass a State & Federal background check (including fingerprints)
- 18yo + active Driver's License / State ID
- Reliable transportation to all scheduled visits
- High School diploma or GED
- Active STNA (State Tested Nurse Aide) or CNA certification, or previous home health aide certification
- Familiarity with Medicaid waiver services
- Experience with: G-tubes / feeding support
- Experience with: Seizure protocols
- Experience with: Behavioral health or crisis response
- Demonstrated ability to follow a written Plan of Care
- Pro-active observation and excellent electronic documentation

- Immediate start with 20+ hours: part-time, weekday position requiring consistent availability
- Weekdays: daytime and evening shifts
- Local travel within Marion + Richland Counties
- Expanded cases/hours may include adjacent Counties
- Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist
- Range of motion exercises
- Assistance in ambulation or exercises
- Performs personal care activities, including but not limited to: Bathing, Shampooing, Skin care/nail care, Oral hygiene, Shaving, Dressing
- Performs household services essential to health care at home, including but not limited to: Meal preparation/feeding, Laundry, Light Housekeeping
- Assists in the patient in taking medications under the direction and supervision of the Registered Nurse
- Knows and follows infection control policies, including proper hand washing techniques
- Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse
- Documents care provided and completes the forms required for the client's records
- Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager
- Demonstrates safe practice in the use of equipment
- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences
- Treats patients and families with respect
- Maintains confidentiality in all aspects of the job
- Accepts and fulfills assignments with the agency
- Exercise good judgment in accepting assignments
- Participates in required in-service programs to meet compliance requirements

Desired Qualifications
- Experience with home health agency
- 1+ years experience as DSP
- Previous home health experience (1-3 years)
- Up to 45- or 60-minute client travel radius (from your home) is highly desired
- Candidates seeking cases closer to home (<30 minutes) will be considered if necessary
