Job Summary
Implement and monitor plans of care, including functional assessments and behavior support plans, while documenting goals and objectives for social, vocational, and life skills. Provide direct assistance with daily living activities such as personal hygiene, meal preparation, medication administration, and transportation. Facilitate community outings, recreational activities, and healthcare appointments to promote inclusion and independence. Utilize positive behavior support strategies to address challenging behaviors and maintain accurate records, daily progress notes, and incident reports. Collaborate with interdisciplinary teams, healthcare professionals, and family members to develop comprehensive care plans and advocate for the rights and preferences of individuals with developmental disabilities.
Required Qualifications
- High school diploma or equivalent
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- Ability to respond effectively to changing needs and environments
- Demonstrated strong commitment to safety
- Maintain confidentiality, uphold ethical standards, and adhere to agency policies and procedures in all interactions and responsibilities
Desired Qualifications
- Experience in direct care or human services
- CPR and First Aid certification
- Additional training in developmental disabilities, behavior management, or healthcare
- Strong interpersonal and communication skills
- Patience, empathy, and a genuine desire to support persons with developmental disabilities
- Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities
