Job Summary
Conduct ongoing support to help individuals become active participants in self-care, health, safety, and socialization while maintaining a high quality of life. Ensure safe and clean homes by assisting with household chores and participating in them as needed. Follow and document all treatment plans, including residential habilitation goals and medical case notes, to promote independence. Support individuals in meal preparation, medication administration, and transportation for medical appointments and recreational outings. Mentor new Direct Support Staff by demonstrating routines and reviewing training manuals. Adhere to overtime policies and attend inter-disciplinary team meetings to discuss progress and goal achievements.
Required Qualifications
- Qualified NYS drivers' license
- Ability to communicate and understand written and oral directives/information
- Physical Aspects may include but are not limited to lifting and transferring of people (children and/or adults)
- Acting as a second staff support during transportation as needed, inclusive of wheelchair vehicle/lockdowns
Desired Qualifications
- High school diploma or equivalent preferred
- AMAP/First Aid/CPR/SCIP-R Training preferred
- Minimum one (1) year experience supporting people with intellectual and other development disabilities preferred
