Job Summary
Provide peer support services to clients at Stout Street Recuperative Care Center, focusing on housing navigation, public benefits assistance, and vital document acquisition. Deliver mentorship and counseling for individuals in recovery from substance use disorders through individual and group activities to build resilience and agency. Coordinate critical integrated services including healthcare and behavioral health in partnership with internal teams and community partners. Transport and escort clients to scheduled appointments using a CCH vehicle or approved personal vehicle. Conduct ongoing community outreach to monitor recovery-oriented needs and maintain accurate documentation of all services provided in EHR systems.
Required Qualifications
- Basic Life Support Certification required within 30 days of employment
- Must have and maintain in good standing, a Colorado driver's license within 30 days of hire
- Must be a person with lived experience including substance use disorders, homelessness support services, and/or mental health challenges with prior and/or ongoing recovery activity
- Must be able to read computer screens and printed documents
- Must be able to hear well enough to communicate with employees and others
- Must be able to stand
- Must be able to climb/stoop/kneel
- Must be able to lift/pull/push
- Must be able to write, type and use telephones, cellphone and online phone
- Must be able to access and interpret information from computer screens and printed materials
- Must be able to communicate effectively with others
- Must be able to remain calm, think clearly, and apply sound judgement in high-pressure situations
- Must be able to perform the essential functions of this position with or without reasonable accommodation and without posing a direct threat to the health or safety of themselves or others
Desired Qualifications
- Completion of accredited Peer Specialist training program highly preferred
- Bi-lingual English-Spanish and/or ASL preferred
- Prior peer mentoring experience serving individuals experiencing behavioral health challenges and/or homelessness highly preferred
