Job Summary
Perform comprehensive podiatry assessments, evaluations, and follow-up care for patients in Skilled Nursing Facilities, Assisted Living Communities, and Adult Homes across Brooklyn, Queens, and Nassau County. Diagnose and manage a variety of podiatry conditions while developing individualized treatment plans to improve patient outcomes and reduce avoidable hospital readmissions. Collaborate with facility nursing teams and supervising physicians, providing education and support to patients and healthcare staff. Maintain accurate clinical documentation through EMR systems and handle Medicaid/Medicare insurance verifications. Full-time or part-time 1099 position with flexible scheduling ranging from one to three days per week for part-time roles or five days for full-time. Board-certified Doctor of Podiatry with an active New York State MD license, current DEA License, and Medicaid certification required. Minimum two years of experience in podiatry care and treatment essential.
