Job Summary
DOC Outreach Specialist provides in-reach to Department of Correction facilities and pre-release/community reentry support for individuals with substance use disorders, delivering overdose prevention education (including naloxone), connecting participants to MAT, detox/outpatient/residential treatment, housing, benefits, transportation, and other social services; builds relationships with DOC staff and community partners, maintains documentation in Cordata, and coordinates care to reduce overdose risk and support successful community reintegration.
