DMH Field Supervisor

Conduct DMH new client openings by completing all required paperwork and documentation. Perform and schedule caregiver evaluations in the field, providing on-the-job training for new personal assistants. Deliver shift coverage for personal care services, activities of daily living, and household duties while monitoring adherence to the Code of Ethical Behavior and dress code. Execute monthly quality assurance visits to review client records and work performance. Investigate complaints and address home safety issues or non-compliance concerns after reviewing with the Program Manager. Complete general office tasks and follow established departmental policies, procedures, and objectives. Requires reliable transportation, a working telephone, and six months of employment with Phoenix Home Care. Starting pay is $20 per hour.
