Job Summary
Coordinate referrals and establish linkages with community service providers while developing case management plans for assigned youth and families. Conduct eligibility determinations, gain parental and youth agreement, and perform required screenings to prepare summary reports for the Youth Diversion Team. Maintain ongoing family and collateral contact, support safety planning and crisis intervention, and administer standardized screening tools to collect data. Provide direct case management services including interventions to increase treatment completion, transport clients to appointments, and provide translation as needed. Maintain records of activities and client histories in compliance with Medicaid and licensing accreditations. Attend team meetings to coordinate case management and review cases.
Required Qualifications
- BA/BS degree in social work, criminal justice, psychology, counseling or related field
- Valid driver's license
- Access to reliable transportation
