Conduct discharge planning and provide comprehensive medically related social services to patients. Execute MDS, care plans, assessments, and timely documentation while adhering to company standards, best practices, and all state and federal regulations. Requires a four-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or a related human services field.
