Job Summary
Review presenting clinical information, identify resources, and coordinate successful discharge from Level I inpatient settings into community care and outpatient treatment. Connect individuals with ongoing Terros Outpatient services while working with clients, hospital staff, outpatient teams, family members, PCPs, and health plans to ensure effective transitions. Participate in individualized service planning and execution of written discharge plans to reduce readmission risks and improve coordination with providers. Perform activities across both inpatient and outpatient settings, including visits and coordination meetings.
Required Qualifications
- GED/High school diploma
- 4 years of behavioral health employment experience
- An associates degree in the field of behavioral health
- 2 years of behavioral health employment experience
- A bachelor's degree (not in the field of behavioral health)
- 1 year of behavioral health employment experience
- A bachelor's degree in the field of behavioral health
- A master's degree or higher in the field of behavioral health
- Microsoft Word
- Excel
- Outlook

- Valid AZ Driver License
- Valid Arizona driver's license
- 21 years of age
- 4 years driving experience
- Valid unrestricted Arizona Fingerprint Clearance Card (Level One)
- ability to apply for an Arizona Fingerprint Clearance Card within 7 working days of employment
