Job Summary
Coordinate patient discharge appointments alongside the inpatient psychiatric unit, collaborating closely with patients to establish goals for overall treatment. Initiate tasks and communicate effectively with a large team while building ongoing relationships with collateral partners regarding mental health and substance misuse resources in NH. Maintain strong organizational skills and excellent communication with co-workers, staff, clients, and community partners to enhance available resources. Requires at least one full year of mental health field experience, a bachelor's degree in psychology or related field, and knowledge of local resources.
