Discharge Planner

Coordinate the discharge process for all CSU clients, including post-discharge follow-up and family meetings to develop and implement after-care plans. Provide and document timely treatment services, assessments, interventions, and aftercare plans while facilitating continuing care exploration for returning home, partial hospitalization, or intensive outpatient programs. Arrange appropriate post-discharge referrals and engage with collateral contacts and providers regarding client needs. Participate in treatment teams to provide collateral information and enter data into the CFBHN SharePoint system as required. Complete all training requirements and other assigned duties.
