Coordinate health care decisions using a systematic approach to improve quality and outcomes while ensuring treatment plans align with patient-related utilization management criteria. Assess psychosocial needs of patients and provide interventions as part of the discharge planning process, managing care across the continuum to promote cost-effective services within allotted timeframes.
