Job Summary
Discharge Planner provides initial discharge planning per CMS Conditions of Participation, collaborating with Case Managers, Social Workers, and healthcare staff to coordinate transitions from hospital to community. Responsibilities include reviewing psychosocial situations, coordinating with community resources and payors, monitoring progress against the discharge plan, delegating insurance-related tasks to Financial Counselors, providing required patient notifications (IMM MOON OON), and ensuring timely, cost-effective discharge planning while maintaining safety and patient-family communication. Requires strong communication, coordination, problem-solving, and the ability to manage multiple priorities in a complex care setting.
Required Qualifications
- High School or equivalent
- Bachelor’s Degree in social work
- 1 year in an acute health care setting (preferred)
- BLS Healthcare Provider (American Heart Association)
- Workplace Violence Training (within 6 months of hire)
- Licensed Vocational Nurse - LVN (CA DCA) preferred
