Job Summary
Discharge Planner and Social Worker needed in a premier skilled nursing facility in Chicago. Responsible for maintaining up-to-date evaluation documentation in compliance with federal, state, and local regulations; developing comprehensive psychosocial assessments; orienting Guests and families to facility services and rights; participating in care conferences; coordinating intake, admission, transfer, and referrals; addressing questions about facility rules; coordinating inventory and labeling of belongings; contributing to interdisciplinary team and care conferences; ensuring timely mandatory documentation; providing counseling and linkage to community resources; developing comprehensive discharge plans in collaboration with Guests, families, and staff; maintaining filing systems and reporting hazards. Requires a minimum of two years’ social work experience (healthcare preferred); bachelor’s degree in social work or related field preferred; open to travel within the surrounding community. Required Qualifications
