Job Summary
Assess client needs and develop individualized discharge plans for adults in the Adult Recovery Unit, addressing mental health, substance use, medical, housing, and social support requirements. Coordinate follow-up appointments, referrals, and community services by collaborating with therapists, nurses, and prescribers to arrange post-discharge care and transportation. Educate clients and families on available recovery resources, including support groups and outpatient treatment options. Maintain accurate documentation in the electronic health record system while ensuring comprehensive transition planning to community care.
Required Qualifications
- A high school diploma or equivalent
- Must be 18 years or older
- Must be authorized to work in the United States
- Must complete background checks and credential verification
Desired Qualifications
- One year experience working with populations served preferred
- Experience and some knowledge and understanding of the needs, problems, and attitudes of persons who experience psychiatric symptoms or challenges with substance abuse
- A general knowledge of the Recovery concept
