Job Summary
Discharge Nurse coordinating discharge planning across a multidisciplinary hospital team to facilitate safe, appropriate and timely discharge for patients with complex needs. Actively coordinates discharge of patients in collaboration with the Discharge Coordinator and Nurse Unit Managers, ensures early discharge planning aligns with length of stay targets, maintains patient confidentiality, informs patients and carers about options, and participates in quality improvement to enhance patient outcomes. Requires strong communication with patients, families, carers, VMOs and external service providers and a self-motivated, reliable professional able to work with minimal supervision. Eligible applicants are registered nurses with AHPRA or have relevant health-related experience, supported by substantial acute clinical experience.
Required Qualifications
- registered nurse with AHPRA or equivalent health-related experience with minimum four years’ current acute clinical experience or recent discharge planning experience
