Job Summary
Coordinate discharge planning for patients with complex needs, working with the Discharge Coordinator and Nurse Unit Managers to ensure early, safe, timely discharge. Actively coordinates discharge processes across a diverse patient cohort, informs patients and carers about discharge options, engages with other service providers for at-risk patients, and participates in quality improvement activities to enhance patient outcomes. Requires being a registered nurse with AHPRA and a minimum of four years’ acute clinical experience, with strong communication skills and the ability to operate independently within a multidisciplinary team.
