Job Summary
Discharge Coordinator/Planner at a skilled nursing facility responsible for coordinating patient discharge plans and arranging post-discharge services. Collaborates with the interdisciplinary team to arrange home health care, rehabilitation, durable medical equipment, and transportation; conducts comprehensive assessments of physical, emotional, and social needs; educates patients and families on community resources and self-care techniques; provides counseling during the discharge process; monitors plan effectiveness and maintains compliant documentation; works with physicians, nurses, social workers, and therapists to ensure integrated care.
Required Qualifications
- Must provide evidence of being free of tuberculosis infection upon hire
- Must read, write, speak and understand English
- Ability to make independent decisions when circumstances warrant
- Must be knowledgeable of laws, regulations, and guidelines pertaining to long-term care
- Attend and participate in workshops to stay abreast of changes in the long-term care field
- Bachelor’s degree in social work, nursing, therapy, or human services preferred but not required
