Discharge call Back Nurse

Discharge call nurse responsible for making and answering calls to discharge patients with empathy and professionalism, following scripted questions to ensure consistency and accuracy. Provides case management in urgent situations, coordinates follow-up to support patient care continuity, educates patients on the HCAHPS survey to improve satisfaction, and escalates priority issues to appropriate resources. Requires RN licensure, BLS, and proficiency with Cerner; English proficiency required with additional languages preferred. Education preferred includes an Associate's of Nursing with Bachelor's preferred; work involves data entry, healthcare customer service, and coordination of post-discharge care.
