Job Summary
Disability Support Practitioner provides one-on-one home and community support to clients aged 18–80 with a range of disabilities, promoting independence and daily living skills. Responsibilities include assisting with daily living skills, personal care (where required), medication support (if trained), behaviour support strategies, documentation, and collaboration with families and allied health professionals. Travel across the North Shore to group homes is required; role focuses on person-centred care, community engagement, and promoting clients’ safety, dignity, and quality of life. Must be available to work Monday–Friday with day shifts 9:00 AM–3:00 PM; requires adherence to individual support plans and contributing to a positive team culture. The role offers opportunities for growth, ongoing specialist training, and progression within CPA’s disability services.
Required Qualifications
- Current NSW Driver's Licence (minimum P2)
- NDIS Worker Screening Check
- Working With Children Check
- First Aid & CPR
- Willingness to work a rotating roster
- Ability to meet the inherent physical requirements of the role
- CPA will support or reimburse relevant compliance certifications
