Director, Population Health

Director of Population Health to lead development and execution of population health strategy across NeighborHealth. This role orchestrates value-based care initiatives, aligns clinical and operational efforts, and partners with IT to build data-driven systems that monitor population health performance. Responsibilities include shaping population health strategy, improving chronic disease management, preventive care, care transitions, and patient engagement; embedding population health principles into primary care workflows; leading cross-department initiatives to address health-related social needs; translating data insights into actionable plans and monitoring KPIs across risk-bearing contracts (e.g., ACOs, MSSP, PCMH); ensuring regulatory compliance and effective payer relationships. The position emphasizes health equity, collaboration with executive leadership, and driving population-level interventions and programs to improve outcomes for defined patient populations.
