Job Summary
Plan, develop, organize, implement, and evaluate social service programs for the facility while interviewing residents or families to obtain social histories. Provide information on Medicare/Medicaid and financial assistance programs, referring residents to external agencies when internal services are insufficient. Coordinate activities with other departments, attend quality assurance meetings, and develop action plans to address identified deficiencies. Keep abreast of federal and state regulations to recommend policy changes and make independent decisions when circumstances warrant. This role requires working beyond normal hours, weekends, and holidays with a 24/7 on-call rotation. Candidates must possess a bachelor's degree in social work or a related field, with an MSW preferred, along with at least two years of healthcare social work experience and one to two years of supervisory experience. Required Qualifications
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Must be a member in good standing with state or national licensing and accrediting bodies.
- Must be able to relate information concerning a resident's condition.
- Must not pose a direct threat to the health or safety of other individuals in the workplace.
- Must have, as a minimum, two (2) year(s) social work experience in a health care setting working directly with individuals.
- Must possess, as a minimum, a minimum of a bachelor's degree in social work or A human services field including sociology, gerontology, special education, rehabilitation counseling, psychology, etc.
- Clinician licensed in North Carolina in appropriate clinic field, e.g. Social Work Director, Psychology Director, Nursing Director.
- Works beyond normal working hours and on weekends and holidays when necessary.
- On call 24 hours a day, 7 days per week.
Desired Qualifications
- One to two (1-2) years of supervisory experience preferred.
- Master of Social Work (MSW) or related field preferred.
