Director of Social Services

Social Services Director is responsible for planning, organizing, implementing, evaluating, and directing the Social Services Department in accordance with federal, state and local standards, and facility policies to ensure the medically related emotional and social needs of patients/residents are met. Duties include coordinating with administration, medical and nursing staff; developing and administering policies and procedures; staying current with regulations; serving on facility committees; planning social services programs; conducting patient/resident assessments, social care plans, and discharge planning; assisting with transportation arrangements; referring to appropriate social services; and informing patients/families about Medicare/Medicaid and financial assistance. Requires a bachelor’s degree in social work or related field and one year of supervised social work experience in a healthcare setting.
