Job Summary
Plan, develop, and organize social service programs at the Health Campus to ensure medically related emotional and social needs are met. Participate in discharge planning and develop written plans of care identifying resident problems, goals, and required interventions. Provide information to residents and families regarding Medicare, Medicaid, and financial assistance programs. Coordinate outside support services and oversee healthcare support plans for chronic diseases, complex conditions, and transitional care from hospital or nursing facilities.
Required Qualifications
- Bachelor's or advanced degree in Social Work or a Human Services field, including but not limited to: Special Education, Rehabilitation Counseling, Psychology
- Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment
- High school diploma or equivalent
- Bachelor's in Psychology
- Licensed practical nurse in Kentucky, Michigan, or Ohio
- Must meet State requirements and regulatory code(s) for licensure or certification
- Bachelor's degree in social work
- Must meet State requirements and regulatory code(s) for licensure or certification
- Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment
Desired Qualifications
- LSW preferred
