Job Summary
Plan, organize, implement, evaluate, and direct social service programs to meet the medically related emotional and social needs of patients and residents. Collaborate with administration, medical, and nursing staff to develop policies, coordinate procedures, and participate in facility committees. Conduct patient assessments, develop care plans, and assist in discharge planning while interviewing families in private settings. Arrange transportation and refer individuals to external agencies when facility services are insufficient. Inform residents about Medicare/Medicaid options and personal property rights. Maintain compliance with federal, state, and local regulations. Assist in community planning related to facility interests and resident needs.
Required Qualifications
- Bachelor's degree or higher in Social Work or Bachelors in a human service field including, but not limited to sociology, special education, rehabilitation counseling and psychology
- One year of supervised social work experience in a health care setting working directly with elderly individuals
