Director of Social Services

Plan, organize, implement, evaluate, and direct the Social Services Department in accordance with federal, state, and local standards to meet residents' medically related emotional and social needs. Coordinate with administration, medical, and nursing staff to develop department policies and procedures. Participate in resident assessments, social care plan development, and discharge planning while interviewing families as necessary. Refer patients to external agencies and arrange transportation when facility services are insufficient. Inform residents and families regarding Medicare/Medicaid options and personal property rights. Maintain rapport with intra-department personnel and community health agencies. Serve on facility committees in the director's absence and assist in community planning related to resident interests.
