Job Summary
Coordinate social services programs for residents by completing charting, psychosocial evaluations, assessments, and care plan entries to meet federal and state licensing requirements. Manage staff through scheduling, mentoring, and hiring while discharging residents, arranging appointments, and coordinating transportation. Lead the Interdisciplinary Team, participate in facility surveys, and maintain the social service directory and resident files in strict compliance with HIPAA standards. Adhere to operational budgets, infection control policies, and safety procedures while reporting occupational exposures immediately. Stay updated on laws and regulations affecting resident care and work cooperatively with all departments and governmental agencies.
Required Qualifications
- Bachelor's Degree in Social Work
- Bachelor's degree in a human services field including but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology
- one year supervised social work experience in a health care setting working directly with individuals
- Minimum of 1 year of experience in social services skilled nursing setting
- Minimum of 1 year of supervisory experience in skilled nursing
- Excellent English language usage in writing, speaking and presenting
- Proficiency with Microsoft Word, Excel, and PowerPoint
- Must be able to move intermittently throughout the day
- Must be able to push, pull, move and or lift up to 25 lbs.
- Must be able to push, pull, move, and/or carry such weight a minimum distance of 15 feet
- Ability to read fine print
