Job Summary
Director of Social Services at Pilgrim Rehab and Skilled Nursing (Integritus Healthcare) leads the Social Services team, develops resident-centered care plans with the interdisciplinary care team, assists with advanced directive planning, advocates for residents and families, attends care plan and Medicare-related meetings, ensures proper documentation for federally mandated requirements, supports census management and admissions processes, and educates residents and families on financial options and transitions of payment options. The role includes orienting new employees and supervising the Social Services team; requires a college degree, experience in long-term care, Massachusetts LSW/BSW credentials, and strong communication skills.
Required Qualifications
- Must possess a college degree
- 2 Years’ experience in a long-term care facility
- LSW, BSW in the state of Massachusetts
- Must be able to read, write, speak, and understand the English language
- Must not pose a direct threat to the health or safety of other individuals in the workplace
Desired Qualifications
- Bachelor's in social work preferred
- 2 Years’ experience in a long-term care facility
- LSW, BSW in the state of Massachusetts
- Must possess a college degree
- Strong communication and advocacy skills
- Ability to read, write, speak, and understand English
- Ability to make independent decisions
- Tactful interaction with residents, families, staff, and external agencies

- Knowledge of financial aspects and insurance verification related to resident care
