Director of Social Services

Direct and oversee all functions of the Social Services Department in a Skilled Nursing Facility; perform psychosocial assessments and develop individualized care plans; coordinate discharge planning and facilitate safe transitions of care; provide counseling and emotional support to residents and families; assist with advance directives and community resources; participate in interdisciplinary care plan meetings and quality improvement initiatives; advocate for resident rights and promote resident satisfaction; ensure regulatory compliance and maintain required documentation; investigate resident concerns and collaborate with nursing, rehabilitation, admissions, and other departments to support resident-centered care; prepare for state and federal surveys and support continuous care improvement.
