Job Summary
Oversee all Social Services functions including admissions, care coordination, transfers, and discharge planning. Serve as the facility's primary discharge planner, ensuring completion of required documentation and screening processes for every resident. Provide direct social work services to assigned residents and families while leading interdisciplinary care plan meetings. Supervise, mentor, and evaluate Social Services staff, ensuring compliance with PASARR Level II reviews and all regulatory requirements. Collaborate with community partners, ombudsman programs, and external agencies to support resident rights and advocacy needs. This leadership role operates within Elderwood at Burlington, a skilled nursing and rehabilitation setting in Burlington, Vermont, where the mission is People Caring for People. The position requires a Baccalaureate Degree in social work and experience with the elderly, with a salary range of $67,860 to $88,218 per year.
Required Qualifications
- Baccalaureate Degree in social work or a related field from an accredited institution
- Casework experience
- Valid Driver's License
