Job Summary
Director of Social Services Lead the social services program at CareOne Brookline. Responsibilities include supervising social service staff, providing direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development, and community relations. Collaborate with an interdisciplinary team, participate in quality improvements, maintain documentation in residents' medical records per policy, conduct evaluations, develop care plans, and coordinate discharge planning. Requires a Master’s in Social Work and current active LSW licensure; ensure adherence to policies, procedures, and ethical standards.
