Director of Social Services

Supervise social service staff and direct a comprehensive program providing support services for residents and families. Conduct initial psychosocial assessments, maintain medical record documentation, and develop social service components of resident care plans. Facilitate discharge planning, coordinate interdisciplinary care conferences, and establish partnerships with the Office of the Ombudsman. Implement peer review processes, update policy manuals, and submit monthly reports to summarize issues. Act as a resource to social workers, assist with recruiting and orienting new personnel, and serve as a liaison between residents, families, outside agencies, and the Center Administrator. Ensure adherence to state and federal guidelines, safety codes, and the CareOne Culture of Excellence Pledge.
