Job Summary
Directs and oversees all social services functions at Bedford Hills Center, supervising staff and coordinating psychosocial assessments, individualized care plans, discharge planning, and community resource coordination. Partners with residents, families, and the interdisciplinary team to advocate for resident rights and promote resident-centered care. Provides counseling and support to residents and families; ensures regulatory compliance with federal, state, and facility regulations; facilitates care plan meetings and drives advocacy and quality-of-life initiatives.
Required Qualifications
- Bachelor's Degree in Social Work, Human Services required
- Minimum of 1 year of supervised experience in a healthcare setting required
