Job Summary
The Director of Social Services combines Social Services and Admissions/Marketing to guide residents from admission through ongoing care, developing care plans, collaborating with nursing, MDS and rehab to facilitate discharge, educating residents and families about rights and health services, maintaining resources for residents, and providing timely referrals to appropriate agencies; requires a degree in Social Work or related field (Bachelor’s or Master’s), long-term care experience, knowledge of admissions/discharge procedures, residents’ rights and documentation, strong communication, organizational skills, and a vehicle with a valid license; role supports professional development, benefits for 30+ hours, and a collaborative, family-oriented workplace.
Required Qualifications
- Bachelor's or Master's Degree in Social Work or related field
- Experience in a long-term care facility
- Knowledge of admissions and discharge procedures
- Familiarity with residents’ rights and documentation requirements
- Ability to communicate with residents, staff and family members professionally
- Valid driver's license and vehicle
- Attention to detail and multi-tasking
- Teamwork and cooperative skills
- Ability to remain calm in stressful situations
