Job Summary
Conduct initial and annual resident assessments evaluating social, psychological, cognitive, and physical needs. Supervise the development and implementation of individual service plans and case management services. Provide supportive counseling to residents and families while coordinating care with outside providers and agencies. Oversee staff training needs and prepare the yearly Resident Services budget. Collaborate with Fund Development to write grants and lead behavioral health and wellness initiatives. Monitor staff activity via Salesforce reports and ensure at-risk tenant issues are identified and reported. Manage a small caseload and participate in community committees advocating for older adults and affordable housing.
Required Qualifications
- Master's degree in Social Work or Mental Health Counseling
- LICSW or LMHC licensure
- At least five years of post-master's experience
- Equivalent combination of relevant education and experience
- Progressive clinical, supervisory, and administrative experience working with vulnerable populations
- Experience with care coordination for older adults
- Ability to travel to all 2Life campuses for meetings and training
