Job Summary

Lead day-to-day operations of high-quality, person-centered care within the community, managing residential placements and ongoing nursing processes while collaborating with physicians and multidisciplinary teams. Supervise assigned care team members, handle hiring and training, and step in as a Med Tech/Resident Assistant when needed to meet licensing requirements. Establish standards for dementia care, ensure compliance with state regulations, and maintain safe inventory of nursing supplies. Provide consultation and phone triage after business hours, manage medication changes and narcotic control, and order labs as directed by primary physicians, hospice, and other providers. Maintain confidentiality, document individual care plans, and ensure equipment preventive maintenance. Work flexible hours as needed to support efficient, cost-effective community operations.

Required Qualifications

  • High School Diploma or equivalent
  • 3 to 5 years of previous experience as an LPN
  • Must have a desire to work with Dementia/Alzheimer's
  • Ability to read, write, and speak English fluently
