Job Summary
The Director of Rehabilitation leads the rehabilitation department (PT, OT, SLP) within a Skilled Nursing Facility, ensuring regulatory compliance (CMS/PDPM/MDS), program development, and interdisciplinary collaboration to drive resident outcomes and financial performance. Responsibilities include operational management, staffing oversight, documentation accuracy, surveys and audits preparation, PDPM case-mix optimization, and oversight of therapy programs for short-term and long-term residents, while fostering staff development and a patient-centered care culture.
Required Qualifications
- Licensed Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) in the state of Ohio
- Preferred 3–5 years of clinical experience in a Skilled Nursing Facility or Long-Term Care setting
- At least 1–2 years of rehab leadership or supervisory experience in SNF/LTC
- Strong working knowledge of PDPM, MDS, Medicare, Medicaid, and managed care reimbursement models
- Experience with rehab documentation systems and EMRs
- Certification in geriatric rehabilitation or specialty programs (preferred)
