Job Summary
Provide strategic leadership and operational oversight for rehabilitation departments across Schofield Residence, Greenfield Health and Rehab, Schofield Home Care, and Schofield Adult Day. Oversee scheduling, staffing, and daily operations while developing policies, monitoring quality outcomes, and ensuring compliance with state, federal, and payer requirements. Lead staff education, performance evaluations, and interdisciplinary collaboration to drive program growth, improve efficiency, and advance the PDPM Medicaid Long-Term Care program. Manage departmental budgets and resource allocation while coordinating shared therapist roles and optimizing home care scheduling. Partner with corporate leadership on strategic initiatives and case management to support continued expansion of rehabilitation services.
Required Qualifications
- Must possess a current license and be registered to practice in the State of New York as a Physical Therapist or Occupational Therapist
- Must be a graduate of an accredited Physical Therapy or Occupational Therapy program
- Minimum of five years of clinical rehabilitation experience
- Minimum of three years of leadership or multi-site rehabilitation management experience
- Must possess the ability to make independent decisions and exercise sound judgment when circumstances warrant such action
- Must possess the ability to deal tactfully and professionally with residents, families, staff, visitors, healthcare professionals, government agencies, and the general public
- Must demonstrate patience, compassion, professionalism, enthusiasm, and a commitment to providing quality care to older adults
- Must be able to communicate effectively with rehabilitation staff, facility leadership, corporate leadership, and other department supervisors
- Must demonstrate strong leadership, organizational, problem-solving, and decision-making skills
- Must maintain positive working relationships with residents, families, staff, and interdisciplinary team members
- Must be able to lead, motivate, coach, and support rehabilitation professionals while fostering teamwork and clinical excellence
- Must be able to manage multiple priorities and provide oversight across multiple rehabilitation programs and locations
- Must be able to maintain confidentiality and comply with all applicable federal, state, local, payer, and organizational requirements
- Must not pose a direct threat to the health or safety of other individuals in the workplace
- Must possess the ability to perform the essential functions of the position with or without reasonable accommodation
- Must be able to move intermittently throughout the work day and travel between organizational programs and locations as required
- Must be able to cope with the mental and emotional stress associated with a leadership position in a healthcare environment
- Must meet the general health requirements set forth by the policies of the facility
- Must demonstrate compassion for older adults and a commitment to promoting quality of life and independence
Desired Qualifications
- Experience in a skilled nursing, long-term care, home care, adult day, or other healthcare setting preferred
- Demonstrated experience overseeing rehabilitation operations, staffing, quality outcomes, productivity, and financial performance
- Strong knowledge of rehabilitation regulations, documentation requirements, billing practices, and PDPM
