Job Summary
Director of Population Health leads strategic oversight of LCDF's population health programs (Chronic Care Management, CCBHC Care Coordination, and Community Health Worker initiatives) to deliver high-quality, patient-centered care, improve health equity, and advance value-based performance under the CEO. Requires a Nursing Bachelor’s degree with 2+ years of clinical health outcomes experience; management experience preferred. Benefits include health, dental, vision, 401(k), and more. Based in Las Cruces, NM as an on-site role.
Required Qualifications
- Bachelor’s degree in Nursing; Master’s degree preferred
- A minimum of 2 years of Clinical Health outcome experience preferred
