Job Summary
Direct pharmacy services programs across nursing home facilities, establishing clinical standards, policies, and operational best practices. Manage relationships with contracted pharmacy providers, oversee formulary management and therapeutic substitution protocols, and implement clinical cost-containment strategies to reduce waste and optimize resident therapy. Lead Pharmacy and Therapeutics committee meetings, analyze medication error trends, and ensure full compliance with CMS, state, and federal regulations. Provide expert clinical guidance to Medical Directors, Directors of Nursing, and facility administrators while conducting up to 25% travel for site visits, quality audits, and clinical reviews. Requires a Pharm.D. or B.S. in Pharmacy with at least five years in long-term care and a BCGP credential.
Required Qualifications
- Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy from an accredited school of pharmacy
- Active, unrestricted registered pharmacist license in the state of operation
- Minimum of 5 years of experience in long-term care (LTC), geriatric pharmacy, or consultant pharmacy operations
- At least 2 years in a leadership or supervisory capacity
- Proven track record in formulary development, therapeutic substitution protocols, and clinical cost-containment programs
- Ability and willingness to travel up to 25% of the time to conduct site visits, quality audits, and clinical reviews across regional facilities
- Exceptional leadership, communication, and analytical skills
- Proficiency in electronic health record (EHR) systems and pharmacy billing platforms
- Background screening through the Florida Clearinghouse

- Board Certified Geriatric Pharmacist (BCGP) credential
