Job Summary
Director of Patient Care Services leads clinical operations for home health, overseeing nursing, therapy, and interdisciplinary staff to ensure care plans, regulatory compliance (CMS CoPs, state licensing, ACHC/CHAP/Joint Commission), OASIS accuracy, and QA/QI processes. The role governs care planning, staff competency, performance improvement, surveys and accreditation readiness, and serves as the primary clinical liaison to physicians, hospitals, and referral partners, while maintaining patient safety, infection control, and high-quality outcomes.
Required Qualifications
- Active RN or MD/DO license in the state of Oregon
- Minimum 5 years of clinical experience, including at least 2 years in home health leadership
- Strong knowledge of home health regulations, CMS CoPs, OASIS, care planning, and interdisciplinary care
- Proven leadership, organizational, and communication skills
