Job Summary
Lead the Social Services Department by supervising team members, overseeing psychosocial assessments, and managing individualized care planning for residents across Skilled Nursing, Assisted Living, Memory Care, and Short-Term Rehabilitation communities. Collaborate with interdisciplinary partners to drive quality improvement initiatives, ensure CMS and Illinois Department of Public Health regulatory compliance, and prepare for regulatory surveys. Support resident advocacy, discharge planning, and connections to community resources while addressing behavioral and cognitive challenges. This role requires a Master's in Social Work, current Illinois LSW license, and three years of senior living experience.
Required Qualifications
- Master's Degree in Social Work (MSW) from an accredited program
- Current Illinois Licensed Social Worker (LSW) license in good standing
- Minimum three (3) years of social work experience in long-term care, senior living, or a healthcare setting
- Minimum two (2) years of supervisory or leadership experience
- Experience working with older adults, including individuals living with Alzheimer's disease and other dementias
- Knowledge of CMS and Illinois long-term care regulations
- Strong interpersonal, leadership, documentation, and communication skills
Desired Qualifications
- LCSW preferred
- Experience working in a CMS-regulated healthcare or long-term care environment preferred
