Job Summary
Conduct initial assessments of prospective residents including off-site visits, perform wellness assessments, and develop individualized care plans. Deliver health promotion services, disease prevention counseling, and self-care education while fostering communication with physicians, healthcare providers, and family members. Hire, train, schedule, and lead the clinical team; coordinate the infection control program and oversee the medication management program. Contribute to quality assurance and performance improvement initiatives while maintaining compliance with confidentiality and privacy regulations. Requires current state nursing licensure (RN preferred), minimum two years supervisory experience, and prior senior/supportive living experience.
Required Qualifications
- Maintains current state nursing licensure
- Minimum of two (2) years nursing supervisory experience
- Must possess leadership and supervisory ability
- Must possess readiness to mentor, motivate, and inspire team members
- Demonstrates exceptional interpersonal skills
- Ability to serve with love, compassion, and dignity
Desired Qualifications
- LPN required, RN preferred
- Prior senior/supportive living experience
