Job Summary
Directs discharge planning for residents at Norfolk Health & Rehabilitation Center, coordinating discharge with supplies, transportation, and services to ensure a safe transition home. Leads five-day discharge planning meetings, assesses patients' support systems and home environments to inform discharge decisions, and coordinates bed management to optimize availability of transitional and extended care. Engages with residents on social issues, conducts customer satisfaction surveys, and collaborates with social services teams (Chaplaincy, Advocates, Responsible Parties) to identify optimal care environments and post-discharge services. Requires a 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or related human services field and at least 1 year of Social Work experience in a healthcare setting. Strong communication, coordination, problem-solving, and teamwork skills are essential.
Required Qualifications
