Director of Clinical Transition

Implement and monitor clinical policies and procedures for assigned communities while ensuring compliance with state, federal, and government agencies. Conduct compliance surveys and participate in state and federal survey activities. Perform regular site visits to supervise operations and provide clinical expertise. Deliver in-service training and orientation to facility staff on survey processes, risk management, and performance improvement. Review and investigate resident complaints as directed, then develop Plans of Corrective Action with facility administrators and directors of nursing. Requires an active RN license, two years of director-level experience, and willingness to travel across multiple states.
