Job Summary
Provide strategic and operational leadership for home health clinical services by overseeing direct and indirect patient care, ensuring regulatory compliance, and managing individualized care plans. Coordinate with physicians and interdisciplinary teams to drive quality assurance, performance improvement, and organizational goals while supporting staff development through mentoring and training initiatives. Monitor operational indicators, census trends, and staffing needs to optimize productivity and assist with budget preparation and resource management. Lead orientation, in-service programs, and continuing education while serving as acting Executive Director during absences. Participate in administrative on-call rotations and foster community relationships to advance exceptional patient outcomes.
Required Qualifications
- Current licensure in the State of Wisconsin as a Registered Nurse (RN), Physician, Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker
- Valid driver's license, reliable transportation, and proof of insurance in accordance with agency requirements
Desired Qualifications
- Three years of management experience in home health, hospice, or a related healthcare setting preferred
- Demonstrated leadership experience supervising clinical and administrative personnel
- Strong knowledge of home health regulations, compliance requirements, and healthcare operations
- Excellent interpersonal, leadership, coaching, and team-building skills
- Ability to analyze operational and financial data to support informed decision-making
- Strong verbal, written, and organizational communication skills
- Ability to build relationships with referral sources, patients, families, and community partners
- Commitment to delivering high-quality, patient-centered care and supporting clinical excellence
