Job Summary
Lead the interdisciplinary team by overseeing direct and indirect patient care services, facilitating care plan discussions, and ensuring timely, accurate clinical documentation. Collaborate with the Clinical Team Coordinator to drive high-quality outcomes, recruit and mentor clinical staff, and monitor compliance with federal, state, Medicare, and accreditation standards. Conduct regular facility visits to support relationships and partner with the Compliance Department to implement staff training initiatives. Manage operational expenses including payroll, supplies, and budget expenditures while supporting quality improvement efforts. This full-time, in-person role requires an active RN license in New Jersey with at least two years of hospice experience and one year of supervisory leadership. Schedule includes Monday–Friday hours with occasional after-hours support.
Required Qualifications
- Active Registered Nurse (RN) license in the state of New Jersey
- Valid driver's license, reliable transportation, and current auto insurance
- Minimum of two (2) years of nursing experience in hospice, home health, or community health
- Minimum of one (1) year of supervisory or leadership experience
- Strong knowledge of hospice regulations, end-of-life care, symptom management, and interdisciplinary care planning
- Excellent communication, leadership, organizational, and problem-solving skills
- Ability to effectively support staff while providing exceptional service to patients and families
- Monday–Friday schedule with occasional after-hours support as needed for clinical operations
Desired Qualifications
