Job Summary
Coordinate the hospital-wide case management program by overseeing review of patient medical records to ensure proper utilization of services and timely discharge planning. Screen all admissions to determine appropriate care levels and supervise case managers to facilitate optimal financial and clinical outcomes. Lead fiscal planning, regulatory compliance, and system analysis to identify improvement opportunities while collaborating with the interdisciplinary team to promote quality of care. Requires RN licensure (or equivalent MSW/clinical licensure) with three to five years of case management experience, preferably in IRF or LTACH settings. Required Qualifications
- RN licensure in the state where the hospital resides
- Social worker licensure or bachelor's degree in social work
- Current BLS certification
- Three to five years of case management experience
Desired Qualifications
- Prior experience in IRF or LTACH
- Prior management experience
- BSN/MSN
- Certification in an approved Case Management Program
