Job Summary
Oversee case management operations, including care coordination, discharge planning, and utilization review. Lead and develop case management staff to support quality outcomes, regulatory compliance, and patient satisfaction. Collaborate with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization. This full-time, permanent role requires an LCSW or RN license, three years of hospital-based case management experience, and a Bachelor's degree or higher.
